"We received a packet to be filled out and returned to the local office. My husband is blind, so I personally walked the packet in with all of the personal information in it. Check stubs, social security numbers, address, etc. The man on the phone told me to drop it in the drop box located in the lobby. I did as I was instructed. Now they are saying that they never received the packet. What in the world is going on over there? I am sure that they are bombarded each day with people and information. However, it its CRUCIAL that they ensure that all information received is logged and processed. It is absolutely unacceptable for a government office to misplace important and private information like that. It would be beneficial to them and to clients to have a time/date stamp provided along with some sort of receipt of any and all paperwork received."
