"I've been trying to send off documents to get my son's a replacement card . I first got the papers to fill out for the replacement card and sent documentation. Well they sent everything back stating the need offical birth certificate. I ended up going to the office and sat there over 2 hours . The lady at the desk was being very disrespectful and nothing was done I asked for the papers to fill out nobody gave me no papers to fill out or anything. I dont approve of the service I got today."

"Having contacted the office by phone and talking with staff about a lost SSAN card. I notified them that I was an US Citizen born abroad, what documents would I need to secure a new card. I was informed just a Driver’s license. After a 20 mile drive and talking to the employee, who did not give his name. I was informed I need my Birth certificate and accompanying documents. Very poor and dispicable service."

How do I report a death?

Funeral homes typically handle reporting of a death to the SSA. However, you will need to provide the funeral home with the deceased’s Social Security number for them to do so. Note that deaths cannot be reported online. If you want to report the person’s death yourself, call the SSA at 1-800-772-1213 (TTY 1-800-325-0778) between 7 a.m. and 7 p.m. Monday through Friday.
